What information does my doctor need to send to the office before I'm seen?
Your doctor will need to provide:

Your demographics

Prior office notes with a history of your symptoms and reason for referral

List of current medications

*Prior diagnostic records relative to the condition

*Prior Pain Management records (if applicable)

What should I bring with me my first day?
Please bring the following to your first appointment:

Your identification. Pre-registration and payment online is required prior to you arriving to the office.

Any medical records or radiology reports you have related to your symptoms

Medications you are currently taking
What happens on the day of my first visit?
You will sit with one of our doctors and have a discussion of your past medical history, your condition, medications and procedures you've trialed in the past as well as other ancillary modalities you've gone through such as nerve blocks, physical therapy, acupuncture, etc. as well as a focused physical exam. We will talk about how our office functions as a way to optimize your quality of life and help manage your symptoms. This can include additional options such as physical therapy, acupuncture, cognitive behavioral therapy, or a referral to an interventional Pain Management specialist while we focus on medication management. We also assess your risk for other conditions that you may be at risk for which will include a review of any Controlled Substances and discussion of drug testing.
